insurance information

Open enrollment has now ended. If you are a new full-time employee, you are eligible for benefits 60 days after your start date. If you are a current full-time employee wanting to sign up for benefits, you will be eligible during next year's open enrollment, with a February 1, 2019 effective date. Anyone wanting to make changes to or terminate their benefits, please contact Shellie Rich at Resourcing Edge for more information. 

MAJOR MEDICAL: If you are looking for a doctor in your network, you will need to say "PCHS Multiplan", managed by a company called Medova Lifestyle Insurance.

MEC PLANS: MEC stands for Minimal Essential Coverage, the company who manages our MEC Plans is under PHCS as well, called Apex Insurance.

VISION, DENTAL, ACCIDENTAL, & CRITICAL ILLNESS: MetLife Insurance covers all Vision, Dental, Accidental, and Critical Illness insurances. Your Dental and Vision cards are below.

GAP Plan: Helps cover major medical deductibles in the event you are hospitalized for any reason. Find how to log in below:

Assurity Basic Care Hospital Indemnity: Will pay your per day you are admitted into a hospital. Gives up to $150 credit with Wellness Rider. More info below:
